MSH International vs Cigna Global.
MSH International and Cigna Global both build international health insurance from a core you add to. MSH’s First’Expat+ starts with hospital cover and standard assistance, then stacks routine care, dental and maternity options; Cigna adds outpatient care, evacuation and other modules to a medical core. The differences sit in evacuation and in how the cover area is set.
In brief
Which is better, MSH International or Cigna Global?
Neither is better for everyone. It depends on what you need the policy to do.
- MSH International suits you if you want hospital cover with standard assistance included, then add routine care, dental and maternity in steps. Repatriation to your country of origin is in the optional Premium Assistance.
- Cigna Global suits you if you’d rather add each benefit as a module, evacuation included, and choose the treatment area yourself. Check that the final quote lists every module you compared.
- Neither, for now, if your current policy already accepts your medical history. Both decide on your medical answers, so have the new terms in writing before you cancel anything.
These are editorial selection pointers, not a personal recommendation or a quote. Your residence, medical history and written acceptance terms can change the shortlist.
MSH International vs Cigna Global: the key differences.
What we compared: First’Expat+: Quartz through Diamond, and Global Health Options: Silver, Gold and Platinum.
| Feature | MSH International · sources | Cigna Global · sources |
|---|---|---|
| Plan structure | A hospitalisation core (HOSPI) with standard assistance, then stacked options: HEALTH, HEALTH+ and HEALTH+CHILD, each at one of four levels. | A core medical plan with optional benefit modules. |
| Outpatient care | Routine care is the HEALTH option. Dental and vision require it, and maternity requires both. In the USA zone, HEALTH is the minimum purchase. | Additional International Outpatient module. Some named core benefits already include outpatient treatment; read both sections. |
| Medical evacuation | Standard assistance, included with hospitalisation, covers emergency transport to the nearest facility with appropriate care and return to your country of residence. Europ Assistance decides whether, where and how; conditions treated in hospital in the six months before a request are excluded. | Optional International Medical Evacuation module, subject to medical assistance approval and terms. |
| Medical underwriting | A health questionnaire for each person, then medical approval, special conditions or refusal. Pre-existing conditions are excluded unless expressly accepted. No moratorium route is described in the documents reviewed. | The individual documents use medical disclosure and any agreed exclusions. Do not import the underwriting options of a Cigna employer scheme. |
| Treatment countries | Five zones; your main country of residence sets the minimum. Your country of origin is covered for stays of up to five months a year in total, and emergencies elsewhere on temporary stays under 60 consecutive days. | Select the treatment area and check residence eligibility. Emergency cover outside that area is a separate, limited benefit. |
| Renewal and continuity | Annual membership renewed automatically unless either party terminates. Group premium rates may be reviewed each 1 January, and membership ends on a permanent return to your country of origin. Check the written terms. | The CGIC February 2024 example protects against claims-based non-renewal, subject to clause 7, while renewal is offered on stated terms. Check the rules for your issuer. |

What the differences mean for you.
Both build on a core, in different steps
MSH starts with HOSPI, hospital cover with standard assistance, then stacks HEALTH (routine care), HEALTH+ (dental and vision) and HEALTH+CHILD (maternity), each at one of four levels; dental and vision need HEALTH, and maternity needs both. Cigna starts with a medical core and adds an International Outpatient module and other modules; some named core benefits already include outpatient treatment. Compare the finished set-up, not the core.
Evacuation: standard assistance at MSH, a module at Cigna
MSH’s standard assistance, included with hospital cover, covers emergency transport to the nearest facility with appropriate care and return to your country of residence. Europ Assistance decides whether, where and how, and conditions treated in hospital in the six months before a request are excluded. Repatriation to your country of origin is in the optional Premium Assistance. At Cigna, international medical evacuation is an optional module, subject to approval by its medical assistance service.
Zones set by where you live, or an area you choose
MSH has five zones, and your main country of residence sets the minimum one. Your country of origin is covered for stays of up to five months a year in total, and emergencies elsewhere on temporary stays under 60 days in a row. With Cigna, you choose the treatment area and check that the plan can be issued where you live; emergency cover outside that area is a separate, limited benefit.
Underwriting in writing, renewal as a membership
MSH asks each person for a health questionnaire, then accepts, sets special conditions or refuses; a pre-existing condition is excluded unless expressly accepted. Cigna’s individual documents use medical disclosure and any agreed exclusions. MSH sells cover as a membership of a group plan: it renews each year unless either party ends it, group premium rates may be reviewed every 1 January, and membership ends if you return to your country of origin for good. The Guernsey Cigna rules cited here are an example for that issuer only.

Who each one suits.
MSH International
Worth examining if a hospital core with standard assistance, and the routine-care options you add, fit the life you lead. Check the zone your residence puts you in, the five-month limit on stays in your country of origin, and whether you need Premium Assistance.
MSH International reviewCigna Global
Worth examining if you want to choose outpatient, evacuation and other cover one module at a time, in the treatment area you pick. Make sure the final quote includes every module you compared.

This page compares MSH International’s First’Expat+, Quartz through Diamond, an individual membership of the ASFE open group plan insured by Groupama Gan Vie, as described in its 2026 information booklet, its June 2026 summary of benefits outside the USA and its Standard Assistance notice, with Cigna Global Health Options individual cover in the February 2026 UK customer guide. The separate CGIC February 2024 Guernsey rules are a labelled renewal example, not universal Cigna terms. The MSH USA zone, Relais’Expat+, Cigna group and Close Care plans and employer schemes are outside it. MSH’s documents name different assistance insurers (Chubb in the assistance notice, Europ Assistance in the healthcare booklet): ask MSH which applies.
