The evacuation split is the headline. On Bupa paper, evacuation is core on every tier, paid in full. On Cigna paper it is a module you must remember to buy — and every evacuation needs advance approval from the assistance service. For a life in Jakarta, Manila or anywhere serious cases travel, this single row outweighs everything else on the page: a Cigna build without the evacuation module is a different product from any Bupa tier, whatever the premiums say.
Granularity versus completeness. Cigna’s seven deductible levels and 30% cost-share options let a disciplined buyer tune the premium like an equaliser — and let an undisciplined one quietly strip the plan to a shell. Bupa’s tiers move in whole steps: less tunable, harder to hollow out. The question is not which system is smarter; it is which failure mode is more likely to be yours.
The renewal clauses deserve reading, not trust. Both carriers state that non-renewal will not be claims-based, and neither publishes an unconditional lifetime guarantee. Cigna’s policy rules open the renewal clause with “If we determine to renew” — five words we quote because no ranking page does. Bupa’s brochure states automatic renewal with no claims-based cancellation, and stops there. For cover you intend to hold at seventy, this paragraph is the comparison.
Regulator geography is not trivia. A Bupa policy is underwritten under UK (or Irish, for the EEA) regulation; Cigna Global’s individual product sits with a Guernsey entity. Both are real regulators; they are not the same regime, and complaint routes, compensation schemes and policyholder protections differ. Cross-border buyers comparing these two are almost never told this.

They differ structurally: Bupa Global includes evacuation and repatriation as core benefits, paid in full, on every tier of its Global Health Plans. On Cigna Global Health Options, evacuation is an optional module purchased separately, and the customer guide requires all evacuations to be approved in advance by the medical assistance service. Do Bupa Global or Cigna Global offer moratorium underwriting?
Neither publishes a moratorium option in the materials we read — both underwrite with a medical questionnaire and full disclosure, with Cigna noting possible special exclusions and limited maintenance cover for some declared conditions.
